Do you have an advance care directive yet? This legal document describes the types of medical care you want and don’t want to receive (including end-of-life activities) and who can speak for you if you are unable to make medical decisions for yourself. Give yourself and your family peace of mind by getting it done!
Caseworkers from Hospice of Santa Barbara will be at the Museum to help you complete your plan. There will be a notary onsite in the Museum’s Fleischmann Auditorium providing free notary services to help you finalize it. Hospice staff will shepherd your plan to Cottage Health, so it’s on file and ready when needed.
